Why does my hand warmer heat up without me snapping the metal disc?

I recently bought a sodium acetate hand warmer, I boiled some water and melted the crystals, then remove it from the boiling water and leave to cool. But it won't cool down and started to heat up without me snapping the metal disc. Anyone knows why is this and how to stop this from happening?
